Education On Gestational Diabetes

You are 27 weeks pregnant. Your health care provider has just told you that you have gestational diabetes. Should you be concerned about gestational diabetes?

The short answer: yes. Good care means a lot for your health and your baby's health.

What is gestational diabetes?

Pregnant women who have never had diabetes before but who have high blood sugar (glucose) levels during pregnancy are said to have gestational diabetes. Gestational diabetes affects about 4% of all pregnant women - about 135,000 cases of gestational diabetes in the United States each year.

We don't know what causes gestational diabetes, but we have some clues. The placenta supports the baby as it grows. Hormones from the placenta help the baby develop. But these hormones also block the action of the mother's insulin in her body. This problem is called insulin resistance. Insulin resistance makes it hard for the mother's body to use insulin. She may need up to three times as much insulin.

Gestational diabetes starts when your body is not able to make and use all the insulin it needs for pregnancy. Without enough insulin, glucose cannot leave the blood and be changed to energy. Glucose builds up in the blood to high levels. This is called hyperglycemia.

Gestational diabetes affects the mother in late pregnancy, after the baby's body has been formed, but while the baby is busy growing. Because of this, gestational diabetes does not cause the kinds of birth defects sometimes seen in babies whose mothers had diabetes before pregnancy.

However, untreated or poorly controlled gestational diabetes can hurt your baby. When you have gestational diabetes, your pancreas works overtime to produce insulin, but the insulin does not lower your blood glucose levels. Although insulin does not cross the placenta, glucose and other nutrients do. So extra blood glucose goes through the placenta, giving the baby high blood glucose levels. This causes the baby's pancreas to make extra insulin to get rid of the blood glucose. Since the baby is getting more energy than it needs to grow and develop, the extra energy is stored as fat.

This can lead to macrosomia, or a "fat" baby. Babies with macrosomia face health problems of their own, including damage to their shoulders during birth. Because of the extra insulin made by the baby's pancreas, newborns may have very low blood glucose levels at birth and are also at higher risk for breathing problems. Babies with excess insulin become children who are at risk for obesity and adults who are at risk for type 2 diabetes.

Treating gestational diabetes


Because gestational diabetes can hurt you and your baby, you need to start treatment quickly. Treatment for gestational diabetes aims to keep blood glucose levels equal to those of pregnant women who don't have gestational diabetes. Treatment for gestational diabetes always includes special meal plans and scheduled physical activity. It may also include daily blood glucose testing and insulin injections. You will need help from your doctor, nurse educator, and other members of your health care team so that your treatment for gestational diabetes can be changed as needed.

For you as the mother-to-be, treatment for gestational diabetes helps lower the risk of a cesarean section birth that very large babies may require. Sticking with your treatment for gestational diabetes will give you a healthy pregnancy and birth, and may help your baby avoid future poor health.

Gestational diabetes -- Looking ahead


Gestational diabetes usually goes away after pregnancy. But once you've had gestational diabetes, your chances are 2 in 3 that it will return in future pregnancies. In a few women, however, pregnancy uncovers type 1 or type 2 diabetes. It is hard to tell whether these women have gestational diabetes or have just started showing their diabetes during pregnancy. These women will need to continue diabetes treatment after pregnancy.

Many women who have gestational diabetes go on to develop type 2 diabetes years later. There seems to be a link between the tendency to have gestational diabetes and type 2 diabetes. Gestational diabetes and type 2 diabetes both involve insulin resistance. Certain basic lifestyle changes may help prevent diabetes after gestational diabetes.

Losing weight Are you more than 20% over your ideal body weight? Losing even a few pounds can help you avoid developing type 2 diabetes.

Making healthy food choices Follow simple daily guidelines, like eating a variety of foods including fresh fruits and vegetables, limiting fat intake to 30% or less of daily calories, and watching your portion size. Healthy eating habits can go a long way in preventing diabetes and other health problems.

Exercising Regular exercise allows your body to use glucose without extra insulin. This helps combat insulin resistance and is what makes exercise helpful to people with diabetes. Never start an exercise program without checking with your doctor first.

Keeping worry in perspective

While gestational diabetes is a cause for concern, the good news is that you and your health care team - your doctor, obstetrician, nurse educator, and dietitian - work together to lower your high blood glucose levels. And with this help, you can turn your concern into a healthy pregnancy for you, and a healthy start for your baby.

When it Rains it Pours, and I am NOT Talking About the Weather

I failed miserably! I got thre results back from my 3hr Glucose test and I FAILED! My levels started out well enough but plummeted the last two hours. I am just devastated. My Doc put in a referral for me to join the "Sweet Success Program" for Gestational Diabetes - isn't that name just rich? "Sweet" - are you kidding me? Umm, hello sweets and diabetes DON'T mesh! Such an oxy-moron! I called, but it was already after 5pm so I will have to call back tomorrow after 8:30am.

On top of all this, I am borderline anemic, I flubbed my EDD paperwork so no "timely rent" will be paid this month and my Grandpa took a nasty spill and is in the hospital and NOT faring well at all! I just can't possibly take another piece of bad new this week!!!!! Give a prego a break! Oh and I have a narly head cold. So, I want to cry just to get all this frustration out, but then I have to deal with a clogged head and that just makes me want to cry more.

I just can't believe I have Gestational Diabetes! I have not gained enough weight to cause myself harm, so WTF? I know this is not the end of the world, but it is just a major blow and creates a list of worries I never anticipated! I am just so angry.

I failed!

I went in for my 1 Hour Glucose Screening test today believing I would pass with flying colors considering I haven't gained a huge amount of weight and hardly ever eat more then I normally did prior to getting prego. Well, I guess I was wrong. My numbers came back at 143 and the normal range is 70-140....so I was labeled "high." I have no clue what this means. My next drs. appt. isn't until next week so I am not sure what my results will mean until then. I will most likely have to take the dreaded 3 hour test - which is just poor torture for any prego!!!! I mean, fasting and then having to sit around for an additional 3 hours is just INSANE! I really hope I am not going to have G.D.!

Its All About the Baby

I am finding there just isn't much sympathy out there for my woes...well except for my Hubby. He knows how much I am going through to grow our lil B and makes sure I get soothed just a little - lol. After all - once the baby gets here, it will be alllll about him, so why not play-cate a mommy to be just a little!

The last two weeks have been a roller-coaster! I hit 6 mos with flying colors and just kept on chugging! This week, Drew changed positions. And let me tell to you - that was no day at the beach for mommy! Man, did it hurt and make me one uncomfortable cookie. And to think, he is just going to get bigger and MORE active in there! He went from being Breech Transverse (feet down facing forward) to what is my best guess - now being Breech facing backwards! So, he is currently jumping on my bladder and spine like it is a trampoline! I am so thrilled as a mom to be entertaining her son with her own body parts - really I am.......

I remember my girlfriend Kristina when she was prego....she used to walk into our office every morning glowing....one morning at about 6 mos she walked in with this scowl on her face....I asked her what was wrong.....and at this point she drops her shoulders and exclaims - 'I'M DONE, I AM JUST FRIGGIN DONE! I DON'T WANT TO BE PREGNANT ANYMORE!" I just chuckled and apologized because I didn't know what she meant - but now I do! Since this pregnancy is just about him and ME getting bigger - you would just rather he be here already so you can get your body and a little bit of yourself back. FAT CHANCE!

Anyway, before I spiral too far - lol! I really am enjoying my lil miracle and my hubby's admiration for it all. The best parts of us are in me and I still can't believe it. We got our first baby-shower gift! Uncle Randall and Auntie Charlotte purchased Drew's mobile and blanky off our registry! They ARE SO CUTE! I assembled the mobile the moment it arrived and LOVE IT! I know he will just be facinated by it! And the blanky is so soft! I look at it and I wonder if it will be THE BLANKET that Drew attaches himself too - we shall see......

6 Mos Milestone

I find it extremely pleasing that I would reach my 6mos milestone on my Hubby's 30th birthday. I can't believe the "boy" I fell in love with at 18 is now 30 and I am now carrying his lil boy in my belly! How much more amazing can life be? I have too many blessing to count!

Last night I couldn't help but feel a little panicked that my Hubby only had hours left in his twenties. I am not one to look at aging as a scary thing. But I just felt a little sad because he was turning 30 and it seems like we were two teenagers in love just yesterday! Time passes to damn quickly. It is so important to live your life with eyes wide open - However, most of us tend to live life with eyes wide shut....I hope I am blessed with at LEAST another 70 years with him! He is just the most special person to me....well aside from his son growing inside of me.

We were sitting on the couch last night and I had pulled my shirt up and off my belly (something I look forward to doing at the end of my day) and I was shocked to see Andrew's lil kick visible!!!! All the books say that this isn't something that happens until after 28weeks - but our lil Drew is again ahead of the curve! He was popping my belly again and again - it was amazing! Unfortunately, Jerry didn't see it, but he sure as hell felt it! I love sharing tummy time with him. I never thought I would like my Hubby rubbing any fat on my belly - but now it is the most soothing thing in the world!

This week has just been full of so much and it is only Wed......Saturday, was our 1yr anniversary of discovering our 1st pregnancy was no longer viable. And a year ago Monday, I was preparing for surgery to have my lil angel separated from my body - but not my soul. I can't believe it has been a year already. It is true that my scars have faded from the surface, but underneith it all - they are alway there. Being pregnant now - doesn't replace what should have been - but it makes me thankful even more for what I have! And with so many of my fellow Angel-Mommies that I met exactly a year ago getting prego along side of me now - I can't help but feel stronger and more confident that we ALL will get what our hearts desire the most - to be mommies!

I spent the weekend surrounded by family and laughing and day dreaming of the days to come. But with it all, I still didn't forget to honor the little one I will forever hold in my heart and not in my arms. And when my lil Andrew gets here - I will give him the kisses that I so deeply want to give my lil angel.

And thankfully I have Drew's CONSTANT nudges to remind me to smile and keep the faith! I can't wait to see him!
